Interactions

A. Top Drug-Drug Interactions:

1.Nitrates (e.g., nitroglycerin, isosorbide dinitrate): Cialis and nitrates both lower blood pressure. Taking them together can cause a significant and potentially life-threatening drop in blood pressure (hypotension), leading to dizziness, fainting, or even heart attack or stroke. It is contraindicated to use Cialis with nitrates or recreational "poppers" (amyl nitrites).

2.Alpha-blockers (e.g., prazosin, terazosin, tamsulosin): Both Cialis and alpha-blockers relax blood vessels, which can lead to excessive blood pressure reduction when used together. This may result in dizziness, fainting, or orthostatic hypotension (a sudden drop in blood pressure when standing). If needed, your doctor may adjust the dosage or recommend spacing the medications.

3.Other antihypertensive drugs (e.g., atenolol, propranolol, enalapril, lisinopril): Cialis can enhance the blood-pressure-lowering effects of these medications. While this interaction is usually less severe than with nitrates or alpha-blockers, it can still cause dizziness or fainting in sensitive individuals. Close monitoring of blood pressure is recommended.

Other Drug-Drug interactions:

  • Ketoconazole, itraconazole (used to manage fungal infection)
  • Protease inhibitors (used to manage HIV infection) Ex. ritonavir, lopinavir
  • Phenobarbital, phenytoin, carbamazepine (medicines used for fits)
  • Rifampicin, erythromycin, clarithromycin (medicines used for bacterial infections)
  • Other medicines used to improve blood flow to the penis (Ex. sildenafil)

B. Drug-Food Interactions:

Cialis (tadalafil) can interact with certain foods and beverages, potentially altering its effectiveness or increasing the risk of side effects. Here are the key drug-food interactions to be aware of:

  1. Grapefruit and Grapefruit Juice: Grapefruit can increase the levels of tadalafil in your blood by interfering with its metabolism. This may amplify side effects, such as headaches, dizziness, or low blood pressure. Avoid consuming grapefruit or grapefruit juice while taking Cialis.
  2. Alcohol: Drinking alcohol with Cialis can enhance the blood pressure-lowering effects of the medication, leading to symptoms like dizziness, lightheadedness, or fainting. Limiting alcohol intake is recommended while using Cialis.
  3. High-Fat Meals: While Cialis can be taken with or without food, consuming it with a high-fat meal may delay its onset of action. Opt for lighter meals if rapid effectiveness is needed.
  4. Caffeine: Though not a direct interaction, combining Cialis with high caffeine intake might worsen side effects like headaches or increase heart rate in sensitive individuals.

Q.I got a call from a friend who is an intern at a business firm in Macau. He said he saw her last week. I asked what he was talking about. She said he asked for a prescription for Cialis, but I said I wasn't sure.

He said he'd be back to work for a couple of days.

“I told her I needed to see a doctor to see if there were any treatments,” he said. “She said, ‘You have to ask the doctor if it's OK to take these pills and you have to get them to your office.’ ”

The problem is that, although it's a common prescription drug, it's not for everyone. It can cause serious side effects, including:

  • Headache
  • Back pain
  • Indigestion
  • Muscle pain
  • Dizziness
  • Diarrhea
  • Nausea

There are, of course, other possible side effects, and it can be hard to know when to stop, but it's very important to get a doctor's OK on the prescription. That means asking the doctor for a full evaluation before taking any medication, even if it's not in your best interest.

“It's not a medical problem, but it's not the only one,” said Dr. Robert W. Doherty, a urologist at the Cleveland Clinic.

“People need to understand the risks and benefits,” Dr. Doherty said. “We all have these personal issues, and we're all aware of them.”

“The problem is people who take it can come up with side effects, and sometimes they can't get the right treatment,” said Dr. Michael M. Ehrlich, a urologist at Cleveland Clinic.

Ehrlich, a urologist, had seen a few men who were taking Cialis before. He said they were all different.

“They could be very upset, and that was one of the more surprising,” he said.

“The way I found the men on Cialis was they were all taking it for the same thing.”

Some of the men were taking Cialis, others were not. There is no indication that the men on Cialis experienced side effects. They didn't have any known risk factors for side effects, so Dr. W. D. Lee, a urologist at Cleveland Clinic, said they were treated with Cialis.

“The thing is, the men who are taking it for a while have different types of the symptoms they have. And they don't respond to any of the other medicines,” he said. “They don't have any known risk factors for the other treatments, so you can see why they're taking it.”

Dr. Ehrlich said there are other medicines that are not recommended for people with some of these problems. He said some of the others include:

  • L-arginineused to treat angina, which is swelling of the heart muscle. The researchers did not test L-arginine as a treatment for erectile dysfunction.
  • Nitroglycerinto prevent the formation of plaque in the arteries, or plaque in the arteries that narrows the arteries, which are a natural part of the heart. Nitroglycerin has been shown to relax the walls of the arteries and may help to prevent plaque build-up.
  • Calcium channel blockersto block calcium from the body's blood vessels, such as Ciprofloxacin and Imdur, which are used to treat heart problems.
  • Alpha-blockersto treat high blood pressure. Alpha-blockers may work to lower blood pressure.
  • Cialisto treat erectile dysfunction, which is a condition where an erection lasts longer than usual, and sometimes even stops.

Ehrlich said his patients often get side effects, but they're not common. “I don't want people to take their medications, and it's very common,” he said.
